    Sendero: An Extended, Agent-Based Implementation of Kauffman's NKCS Model

    The idea of agents exploring a fitness landscape in which they seek to move from 'fitness valleys' to higher 'fitness peaks' has been presented by Kauffman in the NK and NKCS models. The NK model addresses single species while the NKCS extension illustrates coevolving species on coupled fitness landscapes. We describe an agent-based simulation (Sendero), built in Repast, of the NK and NKCS models. The results from Sendero are validated against Kauffman's findings for the NK and NKCS models. We also describe extensions to the basic model, including population dynamics and communication networks for NK, and directed graphs and variable change rates for NKCS.     The role of personal tutoring in supporting the transition to university: experiences and views of widening participation sport students

    Despite the large and diverse cohorts annually recruited to Higher Education sport programmes in the UK, research exploring sport students’ experiences of transitioning into university is very limited. This study was conducted in response to several years of low retention and progression rates across first year sport degree programmes at a post-92 university in the UK. Through focus groups, the study explored the role played by the personal tutor in supporting effective transition of recently enrolled first year sport students from widening participation backgrounds. Most noticeably, the main contributory factors were found to be the nurturing of social integration and use of student-centred personal tutoring approaches to do so. The study further outlined how many widening participation sport students enter university with negative previous personal tutoring experiences and have limited understanding and misguided expectations of the role. The collective findings provide academic colleagues and university management with evidence of one model of effective support for a successful transition into university. Practical implications for widening participation students studying both sport and other degree subjects are presented, as are future research avenues and study limitations

    Clinical Study Neutrophil Gelatinase-Associated Lipocalin: Its Response to Hypoxia and Association with Acute Mountain Sickness

    Acute Mountain Sickness (AMS) is a common clinical challenge at high altitude (HA). A point-of-care biochemical marker for AMS could have widespread utility. Neutrophil gelatinase-associated lipocalin (NGAL) rises in response to renal injury, inflammation and oxidative stress. We investigated whether NGAL rises with HA and if this rise was related to AMS, hypoxia or exercise. NGAL was assayed in a cohort ( = 22) undertaking 6 hours exercise at near sea-level (SL); a cohort ( = 14) during 3 hours of normobaric hypoxia (FiO2 11.6%) and on two trekking expeditions ( = 52) to over 5000 m. NGAL did not change with exercise at SL or following normobaric hypoxia. During the trekking expeditions NGAL levels (ng/ml, mean ± sd, range) rose significantly ( < 0.001) from 68 ± 14 (60-102) at 1300 m to 183 ± 107 (65-519); 143 ± 66 (60-315) and 150 ± 71 (60-357) at 3400 m, 4270 m and 5150 m respectively. At 5150 m there was a significant difference in NGAL between those with severe AMS ( = 7), mild AMS ( = 16) or no AMS ( = 23): 201 ± 34 versus 171 ± 19 versus 124 ± 12 respectively ( = 0.009 for severe versus no AMS; = 0.026 for mild versus no AMS). In summary, NGAL rises in response to prolonged hypobaric hypoxia and demonstrates a relationship to the presence and severity of AMS

    Collaborative case management to aid return to work after long-term sickness absence: a pilot randomised controlled trial

    Background: Despite high levels of employment among working-age adults in the UK, there is still a significant minority who are off work with ill health at any one time (so-called ‘sickness absence’). Long-term sickness absence results in significant costs to the individual, to the employer and to wider society. Objective: The overall objective of the intervention was to improve employee well-being with a view to aiding return to work. To meet this aim, a collaborative case management intervention was adapted to the needs of UK employees who were entering or experiencing long-term sickness absence. Design: A pilot randomised controlled trial, using permuted block randomisation. Recruitment of patients with long-term conditions in settings such as primary care was achieved by screening of routine records, followed by mass mailing of invitations to participants. However, the proportion of patients responding to such invitations can be low, raising concerns about external validity. Recruitment in the Case Management to Enhance Occupational Support (CAMEOS) study used this method to test whether or not it would transfer to a population with long-term sickness absence in the context of occupational health (OH). Participants: Employed people on long-term sickness absence (between 4 weeks and 12 months). The pilot was run with two different collaborators: a large organisation that provided OH services for a number of clients and a non-profit community-based organisation. Intervention: Collaborative case management was delivered by specially trained case managers from the host organisations. Sessions were delivered by telephone and supported use of a self-help handbook. The comparator was usual care as provided by participants’ general practitioner (GP) or OH provider.
